Nursing Services

Nurses provide care to our patients with a focus of promoting comfort and alleviating pain. In addition to the experience and life lessons our Nurses bring with them, Heart ‘n Home also provides extensive training to ensure our staff feel comfortable and confident in many situations.

Hospice CNA

Often the highlight of a patients’ day, CNAs make a significant difference in patients’ lives. Our CNAs are compassionate people that love to serve. CNAs provide personal care services while always preserving the dignity and privacy of patients.

Medical Social Services

Our Medical Social Services team provides resources to our patients and their family to help them resolve issues and concerns at end of life. Heart ‘n Home hires Social Workers and Bereavement Counselors who want to play a role in helping the entire family and patient to cope with death and potentially reconnecting family members, if needed. Our team also takes care of the patient’s family with grief and loss support for up to 13 months after the loss of their loved one.

Spiritual Care Providers

Our spiritual care is non-denominational and focuses on the overall spiritual health of patients and their families. Spiritual Care Providers receive extensive training on the healing process and a revolutionary vision of treating spiritual pain through the individual’s personal wounds and spiritual care at end of life.

Outreach

Our Outreach Team has a huge impact on how we are seen in the community and with our business associates. We often refer to this team as community educators on hospice and the excellent service Heart ‘n Home provides. Outreach Coordinators focus on building relationships with our partners.

Office Personnel

Office Managers, Volunteer Coordinators, Finance, Compliance/Quality Assurance, IT Personnel, Public Relations, Human Resources and others comprise of this discipline. We often say that we will do everything possible to make sure no patient is turned away regardless of their ability to pay. We are able to say this in part because of our administrative staff who support those who directly care for patients.

Leadership

Many members of our leadership team have traveled across the United States to conferences and continued education trainings. Our leaders help people reach their goals through encouragement and a commitment to a higher standard. Meet our team.

MDs/NPs/Pharmacy

Our Medical Directors’ primary role is to oversee the care of our hospice patients. Additionally, they have an important responsibility of modeling and mentoring our nursing staff on the many different aspects of hospice nursing. Our Nurse Practitioners ensure that all of our patients are thoroughly assessed during a Face-to-Face encounter and that the assessment of the patient is given to certifying physicians. Our Hospice Pharmacist and assistant enhance patient care by monitoring therapeutic outcomes through multiple platforms and recommendations, as well as obtaining pharmacy contracts with local pharmacies.
