Heart ‘n Home Hospice & Palliative Care affirms life by providing emotional, physical, and spiritual support to our patients and to those who love and care for them.
With hospice, you only get one chance to do it right. Our mission emphasizes that focus with the phrase “affirms life.” Our patients are our top priority, likewise their well-being is important to us. Heart ‘n Home ensures that our patients receive holistic care. That they are taken care of completely – emotionally and spiritually, as well as ensuring the physical needs are being met through pain and symptom management.
Our mission shows the love and concern that we have, not only for the patients, but for their families. We also want them to have positive memories of their last moments with their loved one. As well as feel the blessings of being present as they walk their loved one to the gates of eternity.
Heart ‘n Home Vision
Through modeling and mentoring our passion and purpose for staff, patients and families, and communities served, we aspire to be recognized nationally as the school of hospice promoting excellence in life and end-of-life care.
The Heart ‘n Home vision is something that drives us forward in achieving excellence on a daily basis and it encompasses, not only the impact we have on patients and families, but the impact on our employees and the communities of which we are a part. Heart ‘n Home leadership is aware of how instrumental employee satisfaction is and how it can either improve or take away from quality patient care. Employees that know they are valued and important, do not have to worry about the integrity and ethics of a company. They can focus all of their time, energy, and soul into providing the best end-of-life care possible.
Our vision emphasizes the education entrenched throughout Heart ‘n Home culture; not only internally, but externally for our community and even other hospice providers. Heart ‘n Home is striving, not necessarily to be the largest of hospices, but to be a definitive leader in providing quality care.
“Affirming life … every day, every time.”
Our tagline has great intention, meaning, and purpose. It connects the dots in everything we do at Heart ‘n Home – from our culture and mission to our vision and quality patient care. At Heart ‘n Home we are striving to do the right thing … every day, every time. This includes valuing and recognizing our employees as our greatest assets. It is important we affirm their lives, as well as the lives of our patients. We strive to do so throughout our education, meetings, and day-to-day interactions. If our employees find joy and peace in their work, we know it positively impacts their ability to provide comfort and legendary service to our patients.
Heart ‘n Home culture is the key contributor to our success in the communities we serve and in our industry. We strive daily to become better within each Value as individuals and as a company. Our mission and Family Core Values, in no particular order, help to define our incredible people and culture. They also ensure our ability to provide excellent care for our patients and families, throughout their end-of-life journey. Incorporating these Values into our daily actions has led Heart ‘n Home to national recognition as a Top 100 Best Places to Work in Healthcare.
