In conjunction with the National Hospice and Palliative Care Organization and the Veterans Administration, we work to ensure excellent end-of-life care is available for our nation’s Veterans and their families. We honor their service and provide support specific to their end-of-life needs and preferences.
America’s Veterans have done everything asked of them in their mission to serve our country and we believe it is never too late to give them a hero’s welcome home. That is why Heart ‘n Home is taking part in the nationally based We Honor Veterans Program.
Our employees use a Military History Checklist to understand and treat the unique needs of each Veteran. Through assessing military service and experiences, Heart ‘n Home professionals are better prepared to meet the specific challenges that Veterans and their families may face at end of life.

Veterans May Experience End-of-Life Differently

We live in a unique time in history, where a very large percentage of our Veteran population has served during a time of war. Every Veteran is different and their unique needs are often influenced by their personal military experience.
