Heart ‘n Home Hospice & Palliative Care can help. The vast majority of Americans say what they want at the end of life is to die in their own homes, as comfortable and pain-free as possible. In fact, a Gallup poll reveals that close to nine in ten adults (88%) would prefer to die in their homes, free of pain, surrounded by family and loved ones: Hospice works to make this happen.

The hospice philosophy is about making sure that a patient’s death experience reflects their wishes in the most dignified manner. At the center of hospice and palliative care is the belief that each of us has the right to die pain-free and with dignity. We also ensure that caregivers and loved ones receive the necessary support to allow this to happen.  Here are five ways that Heart ‘n Home can help:

Heart ‘n Home gives comfort. Our staff are experts at managing life-limiting illness. We ensure that medication, therapies, and treatments all support a care plan that is centered on the patient’s goals. And hospice services can be offered wherever the patient calls home, allowing friends and family to visit freely—something they might not be able to do in a hospital ICU setting.

Heart ‘n Home gives peace. Beyond physical relief, Heart ‘n Home strives to help patients and families find emotional and spiritual comfort during what is often a very traumatic, stressful time. These services are part of the hospice benefit, covered by Medicare, Medicaid, or most private insurances.

Heart ‘n Home gives caregivers guidance. Most families are not prepared to face the death of a loved one. Heart ‘n Home offers services for families and loved ones that provide emotional support and advice to help family members become confident caregivers and adjust to the future, with grief support for up to a year after their loved one passes.

Heart ‘n Home has never turned a patient away for financial reasons.  Hospice is covered 100 percent by Medicare, Medicaid, and most private insurances. Heart ‘n Home assists individuals, with limited financial resources and no health care coverage, in finding support to ensure quality end-of-life care is provided.

Heart ‘n Home understands the unique needs of Veterans at end of life.  We are a proud Level 4 Partner of the We Honor Veterans Program. Many of our own employees are also Veterans.  Therefore, we understand the uniqueness of each Veteran’s military experience and aid in providing peace at end of life. We also assist families and their loved one in accessing their full VA benefits.
