2020 Patient Safety Awareness Week - Heart 'n Home Hospice & Palliative Care, LLC

March 8th-14th is Patient Safety Awareness Week. This initiative was started by the Institute for Healthcare Improvement (IHI) in an effort to increase the safety standards for patients around the world.  On their website, the IHI states:

“Patient Safety Awareness Week is an annual recognition event intended to encourage everyone to learn more about health care safety. During this week, IHI seeks to advance important discussions locally and globally and inspire action to improve the safety of the health care system — for patients and the workforce. Patient Safety Awareness Week serves as a dedicated time and platform for growing awareness about patient safety and recognizing the work already being done.”

Although there have been great strides in patient safety over the last few decades, there is always a need for improvement. At Heart ‘n Home, we take patient safety seriously and take necessary steps to ensure safety for our patients, their families, and our staff.

Heart ‘n Home Staff Trained to Highest Standards

Heart ‘n Home hires only the very best employees and then we spend a great amount of time training them to meet our level of standards. Each employee receives three weeks of initial training at Heart ‘n Home and then has ongoing education and training. After training, the employees are tested to ensure they can complete visits equal to our standards. Heart ‘n Home has a unique visit structure in place to make sure each patient and family receives consistent, safe, compassionate, and thorough care.

Prevention of Medication Mistakes
When a person starts hospice with Heart ‘n Home they can be assured that their medications will be accurately managed.  We have a Hospice Pharmacist that reviews your medications to make sure you are getting the best medications for your situation.  As your needs change, our Nurses and Hospice Physicians will reevaluate your medications. Throughout your care, your medications will be checked and double-checked to make sure you are receiving the proper medications and always have enough medications on hand. Medications for your diagnosis will be provided and are often delivered right to your home.

Preventing Falls
As a person ages and muscles weaken, falls become more common. Heart ‘n Home employees will provide patients and caregivers with education as well as resources to help prevent falls.

Heart ‘n Home employees are very good at anticipating your needs and keeping you fully stocked and prepared for any needs that arise, but we also have nursing staff available 24/7 in case you need help. We are just a phone call away.

Good Communication Between Patient, Family, and Heart ‘n Home Staff
Heart ‘n Home believes that good communication and education helps prevent mistakes. Our employees will take the time to teach the family or caregiver how to completely care for the patient giving them the tools and confidence needed for safe care.
