March 8th-14th is Patient Safety Awareness Week. This initiative was started by the Institute for Healthcare Improvement (IHI) in an effort to increase the safety standards for patients around the world. On their website, the IHI states:
“Patient Safety Awareness Week is an annual recognition event intended to encourage everyone to learn more about health care safety. During this week, IHI seeks to advance important discussions locally and globally and inspire action to improve the safety of the health care system — for patients and the workforce. Patient Safety Awareness Week serves as a dedicated time and platform for growing awareness about patient safety and recognizing the work already being done.”

Prevention of Medication Mistakes
When a person starts hospice with Heart ‘n Home they can be assured that their medications will be accurately managed. We have a Hospice Pharmacist that reviews your medications to make sure you are getting the best medications for your situation. As your needs change, our Nurses and Hospice Physicians will reevaluate your medications. Throughout your care, your medications will be checked and double-checked to make sure you are receiving the proper medications and always have enough medications on hand. Medications for your diagnosis will be provided and are often delivered right to your home.
As a person ages and muscles weaken, falls become more common. Heart ‘n Home employees will provide patients and caregivers with education as well as resources to help prevent falls.
