Heart 'n Home Hospice & Palliative Care provides Hospice Services to patients throughout Idaho's Treasure Valley and surrounding communities as well as Malheur, Baker, and Union Counties in Oregon. All Heart 'n Home Offices are open from 8:30 a.m. to 5:00 p.m. with on-call Nursing Services 24 hours a day, 7 days a week.

Heart 'n Home Hospice & Palliative Care has never turned a patient away, regardless of ability to pay. All memorials and donations are given to the Heart 'n Home Foundation, a 501(c)3 organization. Their mission is to see that all within our reach can pass from this life with dignity and peace.

Heart 'n Home provides each patient with Nurse visits on both a regular and as needed basis. Our Nurses are experts in pain and symptom management and during each visit they assess patient and family needs. Additionally, they communicate and collaborate with the patient's physician, Heart 'n Home Medical Directors, and members of our Interdisciplinary Team to manage medical issues and other symptoms. Our Nurses are on-call 24 hours a day to provide guidance, answer questions, and visit, as necessary. The role of a Heart 'n Home Nurse is to educate and guide patients and families throughout the illness and to provide information needed to understand, cope, and provide care for their loved ones.
