Overview of Heart 'n Home Hospice & Palliative Care
Heart 'n Home Hospice & Palliative Care is a local company established on the rural values of hard work
and a foundation of integrity. Family owned and operated, Heart 'n Home was established in March of
2004 and became Medicare Certified on June 17, 2004. Our core philosophy is to optimize the quality
of life for individuals in the final stages of terminal illness. The Heart 'n Home ethic and focus of
"doing what's right" has allowed us to flourish and grow to be one of the leading hospice providers
in Idaho and Eastern Oregon. This is the epitome of what Heart 'n Home is all about.
Hospice doesn't give the power to shorten or prolong life,
just the power to live it as you choose; to be at home,
if you wish; to be pain-free; to be comforted and cared for.
This is the essence of hospice.

- Heart 'n Home has served over 2,150 patients since its inception in March of 2004.
- Heart 'n Home currently employs a total of 142 full-time and part-time individuals at offices located in Fruitland, Caldwell, Emmett, Meridian, and Baker City.
- Heart 'n Home became Medicare Certified on June 17, 2004 with zero deficiencies.
- Heart 'n Home utilizes technology (e.g., Nurses, Social Workers, and Spiritual Care Providers have laptops for point-of-care documentation) to provide improved continuity of care and enhance our access to patient information.
- Heart 'n Home Nurses have over 700 years of combined nursing experience.
- Heart 'n Home currently has three Certified Hospice and Palliative Care Nurses, and two Certified Hospice and Palliative Care Nursing Assistants.
- The Chief Medical Director at Heart 'n Home is a Board Certified Palliative Care physician.
- Heart 'n Home Social Workers have over 65 years of combined experience in medical social services.
- Heart 'n Home Hospice Aides have provided over 20,150 visits, (including baths, personal care needs, light housekeeping, etc.) for patients in 2010.
- Heart 'n Home has a Board Certified Spiritual Care Providers and three clinically certified Spiritual Care Providers that provide spiritual care and support to patients and families.
- Heart 'n Home provided over 830 crisis care hours in 2010. A time of crisis is a period in which the patient requires continuous care to achieve palliation or management of acute medical symptoms.
- Heart 'n Home provides clinical staff with a minimum of 115 hours of career orientation through the classroom as well as in the field mentoring and training.
- Heart 'n Home feels continuing education is vital for employee knowledge and growth (clinical and non-clinical). Heart 'n Home annually invests an average of $1,180 per employee on state and national education and conferences to increase hospice knowledge, leadership skills, and accountability.
- Heart 'n Home is a Studer Group partner and has received specialized management coaching (2007-2010) from the nationally recognized firm to increase leadership skills and accountability.
- Heart 'n Home has been a member of the National Hospice and Palliative Care Organization (NHPCO) since 2004.
- Heart 'n Home is a member and active participant in the Idaho End-of-Life Coalition. Heart 'n Home’s Director of Outreach serves as the Southwest Idaho Regional Representative on the Board of Directors.

Ontario,
Council,

Boise Metro,
Baker County,
and Gem County
Chamber of Commerce organizations.
- All memorial and community donations are given directly to the Heart 'n Home Foundation, a 501 (c)(3) non-profit organization, which exists to support indigent patients with end-of-life care and family support.
- Heart 'n Home has served 32 indigent or non-reimbursable patients who do not have access to Medicare, Medicaid, insurance, or other financial means, with expenses totaling over $100,000.
- Heart 'n Home has provided several hundred hours of community bereavement, unrelated to hospice patients, since its inception in 2004, and is still continuing to be that support system.
- In 2010, Heart 'n Home provided over 160 in-services to community organizations, doctors offices, assisted living facilities, and skilled-nursing facilities.
- Heart 'n Home utilizes local businesses for Durable Medical Equipment (DME) and pharmacy in an effort to support our community.
- Annually, Heart 'n Home donates a minimum of $10,000 in scholarships to the Cicely Ambassador Team, TVCC, and other local nursing programs.
- In 2011, Heart 'n Home gave back $33,000 in cash donations to support our local communities.
- Heart 'n Home conducts multiple quality assurance surveys to ensure patient satisfaction: NHPCO FEHC (Family Evaluation of Hospice Care) and FEBS (Family Evaluation of Bereavement Services); facility and physician satisfaction surveys; and other surveys and studies specific to current quality assessment and performance improvement needs.
- Heart 'n Home has two additional programs we offer to the community: the Beacon of Light Program reaches out to the faith-based communities and the Cicely Program educates high school students about end-of-life care and gives them an opportunity to serve their community.
- Heart 'n Home has been accredited by the Better Business Bureau for over 5 years!
- Four Heart 'n Home employees have been selected to be part of the "Idaho Hospice Dream Team," which annually honors seven top individuals in the state of Idaho (2008 "Office Support" and "Hospice Aide"; 2009 "Hospice Aide"; and 2010 "Medical Director").
- Heart 'n Home was awarded the 2008 Honoree Health Care Hero for the Elder/Hospice Care category from the Idaho Business Review.
- Heart 'n Home received the Employer Support of the Guard and Reserve "Above and Beyond" award for outstanding service, patriotism, and continued support to national defense in 2008.
- The Fruitland Chamber of Commerce recognized Heart 'n Home as 2006 Business of the Year!
- Heart 'n Home is a partner of the national "We Honor Veterans" program, which reflects our commitment to ensuring Veterans have access to quality end-of-life care!
