Heart ‘n Home uses a personality survey to better understand some of the qualities of each applicant. The personality service is done online and is free of charge. We do not ask every person who applies to do the survey. ONLY complete the personality survey if you have been selected to participate in the interview process. Please follow the instructions below. Personality Survey

  1. Click on the above button and complete the survey. We trust you to be honest and truly want to find the best fit for our applicants.
  2. The results will be in a 4 letter format (i.e. ESFJ), include a basic personality description, and percentage breakdown of each letter. Please include all of the information in the results.
  3. The email you receive from Heart 'n Home will contain instructions on where to copy and paste your results.

Clinical Documentation Exercise

It is vital that our clinicians understand the basic ideas behind charting appropriately for hospice. This skills test allows us to understand the level of competency for each clinician in the documentation area of patient charts.
