Baker City resident, Max Madsen has recently joined the Heart ‘n Home Outreach Team! Outreach Coordinator, Madsen will establish positive community and partner relationships while serving as a resource of hospice knowledge and expertise. He will offer healthcare resources to our community members as well as helpful resources to physicians, facilities, clinics, hospital discharge planners, and other community resources the tools to know when extra support for their patients is necessary.
